Dr. Lohmeyer’s availability for patient examinations spans from Monday to Friday, between 8 am and 5 pm.
Accepted Insurance
When you call please have your insurance information available. We currently take VSP, Spectera, Superior Vision, Medicare/Medicaid, United Health Care, Anthem Blue Cross/Blue Shield, and Cigna.
Patient Forms
In order to streamline the process, kindly choose either the “New Patient” or “Returning Patient” button and also select the HIPAA consent button to complete the necessary paperwork for your examination

Appointment Information
Please bring the following to your appointment:
- Contact lens boxes
- Current glasses
- Health and vision insurance information
- Drivers License
- Copy of your previous prescription, if available
- List of current medications
If you are a new patient to our clinic, please arrive 15 minutes early so that we can review all associated paperwork and get your set up in our system.
